Self-management programs focus on support and empowerment for patients and their families living with a chronic disease * programs are designed to help people make realistic goals, build confidence in their ability in managing their health and maintain an active and fulfilling life
Program also provides workshops for healthcare providers to build skills as self management supports * self-management support is the range of organizational, community and healthcare provider strategies to elicit and support the active participation of people in achieving fulfilling health and wellness
