Service Description:

Provides funding to cover the costs of prescription drugs listed in the Ontario Drug Benefit Formulary/Exceptional Access Program to Ontario residents who have high prescription drugs costs in relation to their income

Fees:

Deductible based on previous year's income

Application:

Apply online, print and mail or request the form at a local pharmacy Trillium Drug Program Application Must re-apply by Sept 30 of every year

Eligibility / Target Population:

To be eligible:

Must be a resident of Ontario
Have a valid health card number
Not already qualified for another drug program such as the Ontario Drug Benefit Program
Not have private insurance that covers 100% of prescription drug costs
Have high perscription drug costs, relative to household income (4% or more of after-tax household income
