Service Description: Diabetes education for children and youth * children, youth, and their families receive interdisciplinary care * follow-up is maintained at regular three month intervals for ongoing support and education * team members involved are nurses, dietitians, social workers, pediatricians, and pediatric endocrinologists

Eligibility / Target Population: Children and youth up to 18 years with diabetes and their families
Languages: English
Language Notes: Provides translation services for several languages, including interpretation support for community members that are deaf or hard of hearing
