Diabetes Education Programs - Kitchener–Waterloo–Wellesley–Wilmot–Woolwich
Diabetes education programs provide individuals with the tools, skills and confidence needed to properly self-manage their diabetes and enable them to live healthy lives. The diabetes education teams consist primarily of a registered nurse and a dietitian who work with family physicians and diabetes care experts. People with diabetes are encouraged to take an active role in the day-to-day management of their own health care and can access one of Ontario's Diabetes Education Programs for assistance.
These services are located outside of Kitchener–Waterloo–Wellesley–Wilmot–Woolwich, but provide service to Kitchener–Waterloo–Wellesley–Wilmot–Woolwich.
