Municipal paramedic services that provide coordinated care in
partnership with local primary care providers and other health partners to support people in their place of residence
Services include:- referrals to community resources
- community paramedic home visit
- community paramedic at clinic
- community paramedic remote care monitoring
- public access defibrillator loaner program
- file of life (fridge magnet)
