Service Description:

Supports for people living with prediabetes or diabetes
One-on-one or group consultation on monitoring blood sugar levels,
medications, healthy eating, exercise, and other lifestyle choices *
certified diabetes educators with registered nurses and registered
dietitians on staff * visits with an endocrinologist (diabetes
specialist) with a referral from your physician
