Service Description: Offers on-site 24-hour personal support services for seniors * provides the Assisted Living program to eligible residents of Bonnie Place Apartments (I and II) and eligible individuals living in the surrounding neighbourhood

Fees: No fee for personal support services * set fees monthly charge for access to the required Emergency Response System
Application: Application must first be made to the housing provider (Bonnie Place) and the applicant must also meet housing requirements (age 65) * then application is made to Assisted Living Services
Eligibility - Population(s) Served: Designed to support high risk seniors, 65 years and older who have complex needs, and who are medically stable
These services are currently available to individuals who:
* live in their own private home, or within a residential setting such as an apartment building within the program boundaries
* demonstrate a need via a Community Health Assessment for daily access to personal support services throughout a 24 hour period (with or without homemaking services) on a frequent basis
* are able to direct their own care or have a substitute decision maker or a live-in caregiver to direct care
* are able to communicate their needs (with or without aides)

* supporting seniors in the surrounding neighborhood with personal care supports
