Service Description: Community health and home care programs, personal support and services that provide specialized care to clients.

South corner of Main St and Sligo Rd intersection

Services include:
  • help with home recovery following hospitalization
  • assistance to cope with long term effects of conditions such as arthritis, stroke, Alzheimers Disease

Programs include:
  • Community Support Services programs (volunteer) - Meals on Wheels, Volunteer Transportation, Volunteer Visiting, Telephone Reassurance
  • SMART exercise program (Seniors Maintaining Active Roles Together)
  • Adult Day Program - for frail elderly and individuals with mild dementia
  • Health Promotion / Education Services - promotion of healthy lifestyles to maintain and improve health at all stages of life
  • Senior Support Worker- in-home support services for seniors to promote independence, wellness, and quality of life
